Description

Summary
The Care Coordinator works in collaboration and continuous partnership with patients, nurses, clinicians and community partners to ensure timely and accurate follow-up for patients. The Care Coordinator assists with labs, patient questions, refill requests and referral follow-up per the PPFA Medical Standards and Guidelines. This role will consistently demonstrate and maintain the "In This Together" workplace values and confidentiality in all communications.

Essential Functions
•Adheres to procedures and requirements regarding the notification, documentation and follow-up of normal and abnormal results as outlined in PPGC Medical Standards and Guidelines.
•Effectively and accurately communicates follow up plans to patients and explains the treatment plan.
•Follows-up on established plans of care for Care Paths and abnormal lab results.
•Provides nonjudgmental communication and excellent customer service for optimal patient experience.
•Sends correspondence to patients via mail and patient portal.
•Collaborates with licensed teammates to answer patient questions in format of their preference (telephone or patient portal).
•Serves as a resource to PPGC teammates regarding follow-up information.
•Performs mandatory STI reporting to the states of Louisiana and Texas, as indicated.
•Works collaboratively with the Care Coordination Team, health centers, providers, and other departments to develop and maintain a person-centered care planning process.
•Demonstrates flexibility and openness in daily work and encourages others to stay open to change and improvement.
•Accepts and readily adapts to changing priorities, new ideas, strategies, procedures, and methods.
•Liaises with other departments, providers and resources to establish and maintain positive working relationships.
•Provides confidential and quality patient care services at all times.
•Makes and encourages positive connections with all co-workers, patients, and community.
•Works and acts independently on-site or remotely with minimum supervision.
•Takes the initiative to proactively resolve barriers, support and or escalate complex follow up cases to licensed teammates.
•Collaborates with Communications to share patient stories, as appropriate.
•Participates in research duties, per protocol.
•Complies with PPGC guidelines and approved standards.
•Other duties as assigned by supervisor.

Qualifications
•At least one year of experience in medical field, preferably in reproductive healthcare.
•Excellent customer service and communication skills.
•Bilingual preferred.

Education and/or Experience
High school diploma/GED required.

•Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.

Mathematical Skills
•Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals.

Reasoning Ability
•Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
•Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Computer Skills
Basic computer literacy - keyboarding, e-mail, word processing, internet research and navigation, calendar, practice management system, and electronic medical records.
