POSITION OBJECTIVE The Certified Home Health Aide (CHHA) performs duties within the assigned Plan of Care for patients and their families/caregivers. The CHHA assists the patient in participating in activities of daily living as appropriate and works as part of the interdisciplinary team. II. ESSENTIAL FUNCTIONS •Provides personal care and homemaking services (light housekeeping) essential to the patient's health care and comfort at home, including shopping, errands, laundry, meal planning and preparation (including therapeutic diets), serving of meals, assisting the patient with activities of daily living and assisting with prescribed exercises and the use of special equipment, as necessary as assigned by the RN Case Manager. •Performs duties as assigned, including vital signs, showers, baths, oral hygiene, nail care, skin care, pressure relief, dressing, feeding transferring (including lifts), and assisting with ambulation, with assistive devices, as needed. •Responds to the physical, mental, social, spiritual, cultural, and emotional needs of terminally ill patients and their families within the scope of their practice. •Demonstrates the ability to use techniques to assist the patient in meeting activities of daily living. •Positions the patient in bed using proper body alignment. •Identifies and communicates changes in pain, symptoms, or status. •Demonstrates the ability to chart according to professional standards. •Provides a safe environment and assures fall, seizure, bleeding, and aspiration precautions are followed as assigned. Assists patient/family with devices that aid speech, vision and hearing, or prosthesis as needed. •Assists patient with elimination, indwelling catheter care, and ostomy care as assigned. •Assists patient with reminders and self-administration of medication as assigned. •Inspects and reinforces wound dressings as assigned. •Provides routine maintenance of medical equipment as assigned. III. OTHER JOB FUNCTIONS •Performs specific work duties and responsibilities as assigned by supervisor. IV. KNOWLEDGE / EXPERIENCE REQUIREMENTS •Must be 18 years of age or older. •Must be fully COVID and flu vaccinated per CDC guidelines and Masonic Village policy. •Must be comfortable working with COVID-positive patients. •Must be comfortable working with pets. V. LICENSES/ CERTIFICATIONS •Current Certification as a Certified Home Health Aide. •Valid Driver's License. VI.
