Hospice Social Worker (LMSW)

Location: Oakland County | Schedule: Full-Time

As a Hospice Social Worker (LMSW) , you will play a vital role in supporting patients and families through the emotional, social, and practical challenges that come with end-of-life care. This role is more than clinical—it’s a calling to provide comfort, dignity, and guidance when it matters most.

What You’ll Need:
  • Master’s or Doctoral degree in Social Work from a CSWE-accredited program

  • Active Social Work license in the state of employment

  • Minimum of 1 year of social work experience in a healthcare or hospice setting

  • Excellent communication, counseling, and interpersonal skills

  • Strong understanding of social work principles and end-of-life care standards

  • Ability to travel locally—50% travel required

  • Capability to sit, stand, bend, and lift/move up to 50–100 lbs

  • Valid driver’s license, reliable transportation, and up-to-date auto insurance

What You’ll Do as a Hospice Social Worker:
  • Conduct initial and ongoing psychosocial assessments to identify patient and family needs

  • Provide emotional support, crisis intervention, and resource coordination throughout the hospice journey

  • Collaborate with the interdisciplinary care team—including nurses, aides, physicians, and chaplains—to support holistic care plans

  • Educate and counsel patients and families regarding end-of-life processes, grief, and available resources

  • Develop, implement, and revise individualized Plans of Care based on patient status, physician input, and evolving needs

  • Ensure all services are provided in accordance with physician orders and agency policies

  • Maintain thorough, timely documentation and communicate findings, changes in condition, and safety concerns to the care team

  • Write and update physician orders and coordinate ongoing care planning

  • Support families with bereavement resources and proper discharge planning
